  • Proven experience as a Behavioral Health Specialist or Clinical Social Worker in a clinic setting

Responsibilities

  • Conduct initial assessments to understand patients’ mental health status and identify their specific treatment needs
  • Collaborate with primary care providers, psychiatrists, psychologists, and other team members to develop and implement personalized treatment plans
  • Provide brief interventions, counseling, and psychoeducation to empower patients
  • Monitor patients’ progress and treatment adherence through regular follow-ups
  • Coordinate care transitions and referrals to specialized mental health services
  • Educate patients and their families about mental health conditions and available resources
  • Maintain detailed and accurate patient records in our electronic health record (EHR) system
  • Participate in case load reviews, team meetings, and quality improvement initiatives
  • Collaborate with external mental health providers and community organizations
  • Adhere to the highest ethical and legal standards

Qualifications

  • Bachelor’s Degree in Sociology or Psychology
  • Proven experience in the behavioral health field
  • Ability to work collaboratively in a team environment
