Job Summary

We are seeking an experienced full stack social worker to join our remote medical case management team. As a social worker, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication to empower members to make positive lifestyle and medical choices.

Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs
• Empower members to make positive lifestyle and medical choices through education and support
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Utilize telephonic and electronic communication to engage with members and provide support
• Maintain accurate and up-to-date records of member interactions and progress

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Masters degree in social work or related field

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and seniors

• Knowledge of community resources and services for medical, financial, and behavioral health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Familiarity with electronic health records and case management software
