Job Summary

Sea Mar Community Health Centers, a leading Federally Qualified Health Center, is seeking a compassionate and experienced Substance Use Disorder Professional II or III to join our Behavioral Health Department in Lakewood, Washington. As a key member of our team, you will provide direct counseling services to patients, perform assessments, evaluations, and intakes, and design individualized treatment plans to support recovery. Responsibilities

  • Provide individual and group counseling services to adults, youth, and their families with primary diagnoses of chemical dependency and emerging mental health issues
  • Conduct assessments, evaluations, and intakes to identify patient needs and develop treatment plans
  • Design, implement, and evaluate individualized treatment plans based on ASAM criteria
  • Coordinate with staff and external agencies to expedite treatment and ensure continuum of care
  • Maintain accurate and current records, including patient progress reports and completion notices
  • Participate in regular staff meetings and in-service trainings for clinical personnel

Requirements

  • Possess a Chemical Dependency Professional license from the Department of Health
  • Must have a Bachelor's degree and a college certificate qualifying as a SUDP
  • Over three years of experience in a related field
  • First Aid and HIV certification required; CPR certification within 90 days of hire
  • Bilingual English/Spanish preferred
