Remote Licensed Social Worker - Telephonic Case Management

As a key member of our team, you will utilize your expertise to assess, plan, and advocate for members with complex needs, empowering them to make informed decisions about their health and well-being.

Key Responsibilities:

• Conduct telephonic assessments and develop personalized plans to address member needs
• Research and provide resources to support members with financial, transportation, safety, and housing needs
• Collaborate with registered nurse case managers and other clinical staff to deliver comprehensive support
• Empower members to make informed decisions about their health and well-being
• Utilize strong communication and interpersonal skills to build trust and rapport with members

Requirements:

• Active and unrestricted LSW, LMSW, LCSW, or LISW license in state of residence
• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work
• Strong communication and interpersonal skills
• Ability to work in a remote, telephonic environment
