Overview

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Hospice Care team on a PRN/Per Diem basis. As an RN in our team, you will be responsible for coordinating and overseeing all aspects of patients' home care, collaborating with patient care teams to develop and implement care plans, and educating families on patient condition, needs, and care.

Responsibilities

  • Coordinates and oversees all aspects of patients' home care
  • Collaborates with patient care teams to develop and implement care plans
  • Educates families on patient condition, needs, and care
  • Performs head-to-toe physical assessments
  • Checks vital signs and manages medications
  • Gives injections, IV therapy, or tube feedings
  • Administers and maintains catheters
  • Completes documentation timely and accurately
  • Supervises LPNs and HHAs
  • Regularly communicates patient progress to the clinical manager, physician, and care team

Qualifications

  • One year of clinical experience as a registered nurse (RN)
  • Current RN license, specific to the state(s) you are assigned to work
  • Minimum six months of home health experience
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
  • Able to assess patient status and identify requirements relative to age-specific needs
  • Familiarity with general use and functions of the computer, including user names and password concepts, internet, email, and navigation of computer desktop
