Adults Social Worker Continuing Health Care (CHC) 6 Month Contract Hybrid Working £32 an hour Hampshire County Council are looking for a Social Worker to work remotely within their discharge to assess team. In this role you will work collaboratively with partner agencies to promote the independence and improve the well being of older adults and adults living in the Hampshire community.
