Home Infusion Pharmacist Nashville, TN
Minimum 1+ years pharmacy experience required. We are seeking a skilled and patient-focused Pharmacist for a home infusion pharmacy located in Nashville, TN. This is a contract opportunity starting in late June or by July 1st, ideal for pharmacists with home infusion or clinical backgrounds looking to join a reputable nationwide infusion provider. Position Details:
- Start Date: Late June / July 1st
- Schedule: MondayFriday, 8:30 AM 5:00 PM
- Rotates to 9:30 AM 6:00 PM shift when on-call
- Type: Full-time, Contract
- Setting: Home infusion (facility and home-based infusion services)
Key Responsibilities:
- Review and approve prescriptions prepared by pharmacy technicians in accordance with state regulations
- Perform final verification on filled prescriptions to ensure order accuracy and timely delivery
- Conduct clinical medication assessments using patient-specific medical data
- Collaborate with pharmacy leadership to ensure pliance with federal/state laws and pany policies, particularly regarding controlled substances
- Support quality assurance and cost containment initiatives related to drug procurement and dispensing
- Respond to clinical questions from internal staff (technicians, nurses) and external providers
- Assess laboratory results and therapeutic drug ranges to ensure optimal patient outes
- Provide leadership support in the absence of the Pharmacist in Charge
- Participate in the on-call rotation as needed
- Maintain strict pliance with HIPAA regulations and protect patient confidentiality
Requirements:
- Graduate of an accredited school of pharmacy
- Active pharmacist license in the state of Tennessee (or willingness to obtain)
- Prior home infusion or clinical pharmacy experience preferred
- Strong munication and multitasking skills
- Proficient puter and documentation abilities
- Ability to work effectively as part of a clinical and operational team
