Provide personalized case management services to Aetna members in Macon, Piatt, or Champaign County, IL. Conduct thorough assessments of member needs and develop individualized care plans. Collaborate with healthcare providers and community resources to ensure effective and timely delivery of services. Monitor and track member progress and make necessary adjustments to care plans. Utilize analytical skills to identify opportunities for improvement and cost savings. Maintain accurate and detailed documentation of all case management activities. Communicate effectively with members, providers, and other stakeholders. Stay up-to-date on industry trends and best practices in case management. Participate in team meetings and contribute ideas for process improvements. Adhere to all regulatory and compliance requirements. Display empathy and compassion towards members and their unique situations. Work efficiently and effectively in a fast-paced, field-based environment. Collaborate with other team members and departments to ensure seamless delivery of services. Uphold Aetna's values and mission to improve the health and well-being of individuals and communities. Continuously strive to meet and exceed performance goals and metrics. Demonstrate strong organizational skills and attention to detail. Maintain confidentiality of member information and adhere to HIPAA regulations. Provide excellent customer service to members and address any concerns or issues in a timely and professional manner.
