Healthcare Case Coordinator

As a Healthcare Case Coordinator, you will be responsible for coordinating the care of CDPHP members who meet criteria for services across the continuum of care. This includes successful transition of care for CDPHP members who have been hospitalized and are being discharged, with a focus on readmission avoidance. You will also develop and implement individualized care plans, collaborate with providers and other healthcare professionals, and provide education and support to members with chronic health needs. To succeed in this role, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will also need to navigate multiple applications and platforms to manage your daily workflow and make data-driven decisions to improve patient outcomes. Responsibilities: • Coordinate care for CDPHP members who meet criteria for services • Develop and implement individualized care plans • Collaborate with providers and other healthcare professionals to ensure seamless care transitions • Provide education and support to members with chronic health needs • Navigate multiple applications and platforms to manage daily workflow Requirements: • Registered Nurse with a current New York State license required • Bachelor's degree in Nursing preferred • Certified Case Manager (CCM) certification preferred • Minimum three (3) years clinical experience required • Experience with chronic condition management and case management preferred Apply Job!
