Occupational Therapist - Integrated Hospital to Home Care Program (Independent Contractor)

Remote · USA Full-time New today

Company Description

Join a growing Allied Health team that’s redefining how care is delivered across the hospital‑to‑home continuum. We’re seeking dedicated, compassionate Occupational Therapists (Independent Contractors) who are excited to work within an integrated care model - one where home care, hospital teams, medical providers, and community partners collaborate seamlessly to support patients in real time, based on their changing needs. If you’re driven to make a meaningful impact in the community, we’d love to meet you. At Spectrum Health Care, you’ll find a supportive and flexible work environment, with opportunities for both in‑person and remote practice, all while delivering high‑quality interventions that help patients achieve their goals. This position bridges inpatient care and community rehabilitation, ensuring safe, effective transitions from hospital to home. You’ll work with patients recovering from surgery, acute illness, complex geriatric conditions, progressive disease, or functional decline - focusing on preventing readmissions, promoting recovery, and maximizing independence. At Spectrum, you’ll practice at the intersection of acute‑care clinical reasoning and community‑based service delivery. You’ll support clients recently discharged from hospital or following post‑surgical pathways, backed by a collaborative interdisciplinary team and a flexible, responsive work environment. Whether you’re a new graduate or an experienced practitioner, we have a place for you. You’ll work with a diverse patient population and have the autonomy to make a real difference in their lives. This role offers the chance to apply strong clinical reasoning, exercise leadership, and contribute to measurable improvements in patient outcomes.
