Epic Analyst (Healthy Planet) - Remote, OH

Interviews will be via Microsoft Teams.Submission Due Date: 06/29/2025Client: State of OhioPosition is full-time remote.Role OverviewThe Epic Analyst – Healthy Planet Population Health plays a vital role in supporting the implementation, optimization and ongoing maintenance of the Epic Healthy Planet module, which is designed to enhance population health management. This role works closely with clinical, operational, and IT stakeholders to ensure data‑driven, value‑based care strategies are supported effectively through Epic. The analyst will help bridge gaps between data insights, care coordination, and clinical workflows, enabling improved health outcomes across patient populations.What You Will DoConfigure, build, and maintain the Epic Health Planet module to support population health initiatives.Collaborate with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.Analyze population health data and metrics to support the design and implementation of targeted care strategies.Develop dashboards and reports to support performance improvement and monitor key quality indicators.Assist in upgrades, testing, and change management for Health Planet and related modules.Maintain up-to-date knowledge of Epic upgrades, functionality, and best practices in population health.Preferred QualificationsEpic certification in Healthy Planet Population HealthExperience in a hospital and/or clinical setting#J-18808-Ljbffr Remote Skills: Analysis Skills, Best Practices, Change Management, Chronic Disease, Data Analysis, Epic Bridges, Epic Healthy Planet, Epic Systems, Hospital, Marconi/MSI Planet, Metrics, Microsoft Product Family, Performance Analysis, Performance Management, Preventive Medicine, Quality Monitoring, Reporting Dashboards About the Company: Ocean Blue Solutions Apply To This Job Apply tot his job Apply To this Job
