Job Summary

As a Case Management Specialist, you will play a critical role in our Double Qualified Special Needs Plan (DSNP) program, working with individuals who are enrolled in Federal Medicare and Medicaid and present with complex health and social challenges. You will use your exceptional communication and interpersonal skills to build strong relationships with patients, families, and healthcare providers, ensuring that each individual receives the highest level of care and support.

Key Responsibilities

* Conduct comprehensive assessments of patients' needs and develop personalized care plans to achieve optimal outcomes

  • Identify high-risk factors and service needs that may impact patient outcomes and collaborate with healthcare providers to develop effective solutions
  • Coordinate and implement care plan activities, monitoring progress and making adjustments as needed
  • Collaborate with case managers, healthcare providers, and other stakeholders to overcome barriers to meeting patient goals and objectives
  • Identify and report quality of care issues through established channels
  • Utilize effective communication and interpersonal skills to engage patients and families in their care, promoting self-advocacy and empowerment
  • Provide education, support, and resources to enable patients to make informed decisions about their healthcare and well-being

Essential Qualifications

* 3+ years of experience in a health-related field

  • Strong understanding of healthcare systems, policies, and procedures
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and collaboratively as part of a team
  • Strong analytical and critical thinking skills
  • Proficiency in case management software and electronic health records

Preferred Qualifications

* Certification in case management (CRC, CDMS, CRRN, COHN, or CCM)

  • Experience working with geriatric special needs, social health, and disability populations
  • Bilingual (English/Spanish; English/Creole) or multilingual skills
  • Knowledge of evaluation, screenings, and care planning
  • Experience working with Federal health insurance and Medicaid programs

Education

* Bachelor's degree or equivalent experience required
