About the Opportunity Support Health is seeking a dedicated Nutritionist or Dietitian to join our Social Care Network (SCN) department. This is a vital role focused on evaluating family needs and determining eligibility for essential services through virtual coaching. Key Responsibilities
- Conduct Virtual Assessments: Lead Zoom sessions with caregivers to analyze dietary patterns, household food access, and medical requirements.
- Determine SCN Eligibility: Review household data to identify nutritional risks and assign families to the correct service categories based on program guidelines.
- Provide Parent Coaching: Offer actionable guidance and health recommendations tailored to each family’s specific challenges.
- Manage Clinical Documentation: Review preliminary screening materials before sessions and submit finalized assessments immediately following the meeting.
- Maintain Standards: Ensure total confidentiality (HIPAA compliance) and maintain a professional, private workspace for all consultations.
The Ideal Candidate
- Qualified Professional: Certified Nutritionist, Dietitian, or a student currently pursuing a degree in a relevant field.
- Strong Communicator: Ability to build quick rapport with parents and provide a warm, supportive presence over video and phone.
- Technically Proficient: Comfortable using Zoom and Microsoft scheduling tools; must have reliable high-speed internet.
- Detail-Oriented: Capable of documenting findings accurately and identifying urgent concerns that require immediate escalation.
