POSITION SUMMARY: The Senior Manager, Chronic Care Management is responsible for leading and overseeing a multi-level team of Data Quality Reviewers (DQRs), Team Leads, and Data Quality Managers to ensure that system-generated clinical data meets established standards for accuracy, completeness, and clinical relevance in preparation for provider review and sign-off. This team is accountable for reviewing high volumes of curated patient data and ensuring that all clinically pertinent information is accurately identified, validated, and communicated to the Advanced Practice Provider (APP) Review Team. The team’s output directly supports onsite and telehealth providers in delivering informed clinical decision-making, enhancing care coordination, and enabling early identification of patients at risk for clinical decline. PRIMARY RESPONSIBILITIES: Provide direct supervision and leadership to Data Quality Managers, with overall accountability for the performance and outcomes of their teams, including Team Leads and Data Quality Lead, develop, and manage a multi-tiered clinical review team, ensuring alignment with organizational goals and performance expectations Ensure the accuracy, completeness, and clinical integrity of all reviewed data prior to provider handoff Drive compliance with CMS and regulatory requirements, including documentation standards for CCM, BHI, and related programs Oversee team productivity, quality metrics, and workflow efficiency to meet volume and turnaround expectations Establish and monitor key performance indicators (KPIs) related to quality, productivity, and timeliness Identify trends, gaps, and opportunities for improvement in clinical data and workflows Partner cross-functionally with Tapestry Leaders, Clinical, Operations, Product, and IT teams to enhance processes and system functionality Support the development and implementation of standardized workflows, training programs, and quality assurance processes Ensure effective communication and collaboration between DQR teams and APP reviewers Promote a culture of accountability, continuous improvement, and high-quality clinical outcomes Protect patient information and adhere to standard of confidentiality with all patient information. Stay abreast of the latest developments in chronic care management and incorporate best practices into team processes. All other duties as assigned. MINIMUM REQUIREMENTS Bachelor’s degree or equivalent Registered Nurse Preferred, may consider LPN with strong background and experience 5 or more years managing large teams to drive operational metrics and performance Experience managing teams in high volume healthcare and proven track record of program development and growth. Experience in long term care, chronic care management, physician practice management and/or remote patient monitoring management. Strong analytical skills including interpreting reports, identifying metrics and data to understand processes, communicating findings, identifying and implementing improvement tactics. PREFERRED SKILLS Strong verbal/written communication and presentation skills Proficiency in Microsoft Office Suite, including Excel, Word, and Outlook Experience using Microsoft Teams for communication and collaboration Familiarity with SharePoint and workflow management tools (e.g., Monday.com) Working knowledge of Power BI for reporting and data visualization High level of emotional intelligence Process improvement methodology experience Experience in working with leadership to promote ideas, issues, and initiatives within a constructive group framework The anticipated salary range for this role is $130K-135K annualized. This remote position follows a location-based compensation structure. The posted salary range represents the potential pay range across various U.S. geographic markets. Actual compensation will be determined based on the candidate’s primary work location, experience, qualifications, and internal equity considerations, in accordance with applicable pay transparency laws.
