Summary

PRIMARY FUNCTION

The Clinical Pharmacist reports to the Enterprise Innovation Pharmacist and provides direct patient-centered medication management services across the Pediatric Associates enterprise. This role focuses on optimizing medication therapy, improving adherence, and supporting value-based care initiatives.

ESSENTIAL DUTIES

Patient Care

· Conduct comprehensive medication reviews and reconciliations.

· Identify and resolve medication-related problems (non-adherence, interactions, duplications, gaps in therapy).

· Provide patient counseling via phone or telehealth.

· Support transitions of care and chronic disease management.

· Develop care plans in collaboration with providers.

Quality & Value-Based Care

· Support Medicare Stars and Medicaid pharmacy quality measures (e.g., PDC).

· Conduct targeted outreach to close medication-related gaps.

· Document interventions per regulatory and payor requirements.

· Assist with formulary alignment and biosimilar transitions.

Collaboration

· Communicate therapeutic recommendations to providers.

· Participate in interdisciplinary care coordination.

· Escalate complex cases as needed.

SUPERVISORY RESPONSIBILITIES

· No supervisory responsibilities

QUALIFICATIONS

· Doctorate of Pharmacy required

· Active pharmacist license in good standing

· 1+ years clinical pharmacy experience in ambulatory care, pediatrics, or managed care preferred but not required

SKILLS

· Strong patient counseling and communication skills.

· Knowledge of chronic disease guidelines.

· Ability to interpret pharmacy data and document interventions.

· Proficient in EMR and Microsoft Office.

WORKING CONDITIONS

· Patient-facing (telehealth).

· May be remote (U.S.-based, HIPAA-compliant workspace required).

· Primarily computer-based with deadlines and outreach targets.
