Position Summary

The Weekend Work From Home, Home Health RN Clinical Coordinator is responsible for carrying out administrative and management functions and oversight in the absence of the Director of Clinical Services. May carry caseload due to ebb and flow of patient census. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies

Position Specific Responsibilities

  • • Promotes a positive working relationship between agency and corporate personnel. • Promotes positive working relationship with family, physician and other facilities. • Attends and participates in staff meetings, educational programs and community events, as requested. • Promotes home health philosophy. • Communicates identified needs and potential solutions to supervisor. • Assists with onboarding, provide mentorship to new staff. • Tracks DME and medical supplies orders and advises management team of trends. • Manages labs/physician orders. Fax to providers as needed. • Submits requests for medical records and hospital inpatient charts, and processes provider notifications when relevant. • Oversees scheduling process. Obtains POSOC as needed. • Answers incoming telephone calls / clinical calls, takes accurate messages, and transfers calls appropriately. • Assists DCS with pre- and post-case conference activities, including agendas, update coordination notes as needed. • Performs RN Case Manager responsibilities as needed and may carry a caseload due to patient census ebb and flow. • Performs other duties as assigned

Education and/or Experience

  • Active and unencumbered Registered Nurse License required.
  • One (1) year of Home Health experience required.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Other Skills and Abilities: Ability to define problems, understand and collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in a variety of forms and deal with several abstract and concrete variables. Proficiency in Microsoft Office Suite. Strong organizational and interpersonal skills. Ability to work a flexible schedule. Ability to maintain confidentiality of information, such as patient and company files. Knowledge of medical terminology.

Certifications, Licenses, and Registrations

  • Current Registered Nurse State License as required.
  • Current CPR certification.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
