Responsibilities:

As a Medical Social Worker with CenterWell Home Health, your primary responsibilities will include:

• Assessing patients' social and emotional needs, and developing a personalized plan of care to address these needs
• Evaluating patients' relationships with their medical and nursing needs, financial resources, and community resources
• Providing direction and supervision to Social Worker Assistants as required
• Collaborating with the interdisciplinary care team to develop and implement patient care plans
• Participating in discharge planning and identifying community resources to support patients and their families
• Maintaining accurate and timely documentation, including case conferences, patient/physician contacts, and visit reports
• Supervising, instructing, and evaluating the performance of Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards

Requirements:

To be successful in this role, you'll need:

Essential Qualifications:

• Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
• Social Worker licensure in the state of practice (required by state law or regulation)
• A valid driver's license, auto insurance, and reliable transportation
• Proof of current CPR certification
• Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice

Preferred Qualifications:

• Experience with discharge planning and community resource development
• Knowledge of medications and their correct administration
• Excellent oral and written communication and interpersonal skills
• Ability to organize tasks, develop action plans, set priorities, and function under stressful situations
• Ability to be flexible in work hours and travel locally
