About the Role

Innovaccer is looking for a Clinical Transformation Consultant to support clinical transformation initiatives by assisting in patient screening and the collection of clinical data for studies. This role plays a key part in ensuring the integrity and accuracy of patient data in alignment with study protocols and healthcare standards.

A Day in the Life

  • Relationship Development: Primary purpose of the role is to support the relationships and constant touch-points for the pharmacy end users adopting and leveraging our platform and tools.
  • Goal and Requirement Understanding: Understand pharmacy customer’s goals, along with requirements and evolutionary needs to align with Innovaccer’s payer and provider solutions and programs. Conversely, understand Payer program goals and align pharmacies to exceed Payer’s expectations for program engagement and outcomes.
  • Value Opportunity Identification and Realization: Identify and communicate opportunities for pharmacy customers and realize value generated from Innovaccer's products and services through supporting clients.
  • Clinical Training and Education: Provide clinical training, demos, and education, including the support of any end user certifications and upskilling required to fully leverage and grow the transformational impact of non-dispensing sources of revenue from a clinical perspective. Provide guidance and training related to medical billing and coding
  • Will provide behavioral change management support to the pharmacy end users and encourage ways to utilize and adopt the platform.
  • Quality Measure: Be the trusted advisor for the pharmacy customer related to pharmacy’s role regarding quality measure performance and revenue generation for quality care gap closures. This role will serve as a Subject Matter Expert for a clinical program, understanding all of the nuances associated with that program.
  • Change Management: Manage the support of changes relative to customer’s utilization of EQUIPP, its data or skill changes to fully utilize Innovaccer products, including those products deeply focused for pharmacy end users.

Identifying and Nurturing Growth Opportunities: With the support of the Customer Success team, continue fostering and developing more opportunities for Innovaccer to grow relationships with its Payer and Pharmacy customers to foster upsell opportunities.

What You Need

    • Bachelor of Science in Nursing/RN required OR Bachelor of Science in Social Work/LCSW required
    • 5+ years of Healthcare/VBC/Population Health Management experience.
    • 5+ years in clinical environment
    • Conversant and experienced in contemporary payment models including value-based care, shared savings, and similar programs
    • Strong clinical skills with demonstrated success in developing or managing clinical pathways.
    • Great work ethic and proven leadership and team-building skills
    • Excellent written, verbal communication skills
    • Ability to develop and present clinical presentations
    • Strong organizational and relationship building skills
    • Ability to nurture relationships within the accounts
    • Strong problem-solving skill
    • Excellent attention to detail and problem-solving ability with demonstrated hands-on work experience in data analytics
  • Preferred:
    • Masters Degree or higher
    • Informatics Certification or Experience
    • Healthcare IT Vendor Experience
    • Experience Working with Partnerships and Alliances
    • Experience Serving as a SME for cross-functional teams (i.e., Strategy, Product Management, Marketing)
    • Consulting experience
    • Experience with medical coding and billing
